We are seeking a highly skilled and compassionate Registered Nurse to join our Primary Care team at Northern Health Authority. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the development of our team.
Job Summary:The successful candidate will be responsible for providing primary care services to patients in our community. This includes conducting assessments, developing and implementing care plans, and collaborating with other healthcare professionals to ensure comprehensive care.
Key Responsibilities:- Conduct thorough assessments of patients to identify their health needs and develop individualized care plans.
- Provide direct patient care, including administering medications, performing procedures, and educating patients and families on health management.
- Collaborate with other healthcare professionals, including physicians, nurse practitioners, and allied health professionals, to ensure comprehensive care.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Develop and maintain strong relationships with patients, families, and community members to promote health and well-being.
- Current registration with the British Columbia College of Nurses and Midwives (BCCNM).
- Three years of recent experience in primary care nursing, with a focus on chronic disease management.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
- Current valid driver's license.
