Key Responsibilities:
The Social Worker will conduct thorough assessments, develop care plans, and coordinate treatment for assigned clients and their families.

Collaborating with primary care providers and the interdisciplinary team, the Social Worker will deliver direct psychosocial support, counseling, group interventions, system navigation, advocacy, education, and connection to specialized care resources, promoting client self-management.

Additional duties include:

  • Delivering direct social work services and care coordination for individuals, families, and groups.
  • Contributing a social work perspective to team practices; engaging in planning and program development; providing operational, policy, and ethical recommendations through consultation and information collection.
  • Maintaining accurate client records through documentation of assessments, treatment plans, progress notes, and reports, adhering to established standards and regulatory requirements.
  • Serving as a resource for clients and families by offering information about community resources, facilitating referrals, and aiding in the development of community support systems.

Work Schedule:
The position requires availability Monday to Friday from 08:30 to 16:30, totaling 37.5 hours per week.
