About the Role

We are seeking a compassionate and dedicated Caregiver to join our team at Transitions Care, Inc. as a Support Worker for Adults with Disabilities. As a Personal Care Assistant, you will play a vital role in providing high-quality care and support to individuals with disabilities, enabling them to live independently and with dignity.

Key Responsibilities
  • Personal Care: Assist clients with bathing, dressing, and grooming, as well as administering medications and providing personal care.
  • Household Management: Perform light housekeeping and cleaning duties, including laundry and meal preparation.
  • Companionship: Provide emotional support and companionship to clients, engaging in activities that promote social interaction and mental stimulation.
  • Transportation: Assist clients with transportation to appointments and outings, ensuring safe and timely arrival.
  • Communication: Maintain open and effective communication with clients, family members, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Education: Secondary (high) school graduation certificate or equivalent.
  • Experience: Will train, but previous experience in caregiving or a related field is an asset.
  • Certifications: CPR and First Aid certificates are required.
  • Language: Fluency in English is essential.
