Clinical Social Worker, Overdose Response Team

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a key member of our Overdose Response Team, you will play a critical role in providing clinical and consultative case management services to individuals and families affected by substance use.
Key Responsibilities- Conduct comprehensive social work and psychosocial assessments to identify client needs and develop effective care plans.
- Provide therapeutic counseling and support to clients and their natural supports, including crisis intervention and harm reduction education.
- Collaborate with interdisciplinary teams to coordinate services and facilitate communication around client care.
- Develop and maintain effective relationships with community agencies and stakeholders to enhance system processes and resources for clients.
- Participate in program evaluation and quality improvement initiatives to identify opportunities for change and improve client outcomes.
- Bachelor's degree in Social Work from an approved school of Social Work.
- Current full registration with the BC College of Social Workers.
- Two years recent, related experience in a complex healthcare environment, including one year experience working with clients who have been systematically marginalized with multiple health issues.
- Valid B.C. driver's license and access to a personal vehicle for business-related purposes.
- Current First Aid and CPR Level C.
- Demonstrated ability to work with priority and culturally diverse populations related to substance use and the toxic drug-poisoning crisis.
- Knowledge of legislation and government policies and procedures that relate to substance use practices and how they may affect client care, resource access or practice guidelines.
- Crisis prevention/intervention, problem-solving skills and conflict resolution/de-escalation skills.
