Social Services Coordinator

Job Summary
The Social Worker at The EMP is a vital member of our interdisciplinary team, responsible for delivering high-quality care to patients and their families. This role requires a strong understanding of the target population and available community services.
Key Responsibilities
- Coordinate and provide quality care within the scope of practice.
- Develop and maintain relationships with patients, families, and community resources.
- Assess patient needs and develop individualized care plans.
- Collaborate with healthcare professionals to ensure comprehensive care.
- Provide education and support to patients and families.
- Document patient progress and maintain accurate records.
Requirements
- Bachelor's degree in social work from an approved school.
- Member in good standing with the New Brunswick Association of Social Workers.
- Valid New Brunswick Class 5 Driver's License and good driving record.
- CPR Level C certification.
- Excellent communication and interpersonal skills.
- Ability to work efficiently as a team member and possess leadership skills.
- High regard for confidentiality.
