Job Summary:

We are seeking a highly skilled and experienced Health Services Practitioner to join our team at Norview Lodge in Simcoe, ON. As a key member of our interdisciplinary team, you will be responsible for providing comprehensive primary care to our residents, including assessments, medication management, and palliative care.

Key Responsibilities:

  • Collaborate with the Medical Director/Physician and the Manager of Nursing and Personal Care to provide resident care.
  • Participate in the Infection Prevention and Control, Medical Advisory, Pharmacy and Therapeutics, Responsive Behaviour, Palliative Care/Pain, Skin and Wound Care, Continence Care, Fall Prevention and Management, Restorative Care, and Continuous Quality Improvement committees.
  • Attend the home on a regular basis to provide comprehensive primary care, including assessments for residents as part of the interdisciplinary healthcare team.
  • Perform person-centred health assessments of residents on admission, annually, and as needed, including three-month medication reviews.
  • Participate in providing after-hours and on-call coverage.
  • Advocate for and provide palliative and end-of-life care.
  • Seek consultation with the Medical Director/Physician when a diagnosis or treatment plan is unclear or beyond the scope of the Nurse Practitioner.
  • Undertake all duties and responsibilities in compliance with Health and Safety standards, guidelines, and legislation.
  • Conduct performance management of Registered staff.
  • Perform admission and annual resident examinations and complete written reports of the findings.
  • Order and/or perform appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up.
  • Diagnose acute and chronic health conditions.
  • Manage resident care by providing pharmacological, complementary, and/or counseling interventions and perform procedures within the NP scope of practice.
  • Document clinical data, assessment findings, diagnosis, plans of care, therapeutic interventions, resident responses, and clinical rationale in a timely and accurate manner.
  • Participate in resident care conferences.
  • Increase continuity of care through collaboration, consultation, and referral as appropriate with the healthcare team.
  • Assist, support, guide, and provide consultation to the home regarding challenging resident clinical situations.
  • Support and advocate for the resident's best interests while engaging the resident/family/substitute decision-maker regarding care.
  • Provide formal and informal training, guidance, and coaching in the management of clinical care to the interdisciplinary team members, serving as a resource person.
  • Complete referrals to specialized consultants, services, and other healthcare providers.
  • Collaborate with the interdisciplinary team related to resident plan of care.
  • Adhere to infection control policies, procedures, protocols, routines, and additional practices as required.
  • Participate in continuing education activities, in-service educational opportunities, and staff meetings.
  • Participate in annual mandatory education as per MOHLTC regulations.
  • Maintain an awareness at all times of emergency procedures and participate in drills/exercises and associated educational opportunities.
  • Maintain confidentiality at all times, safeguarding Personal Health Information and Personal Information.

Requirements:

Knowledge and Experience:

  • Registered Nurse with a current Certificate of Competence with the Ontario College of Nurses-Extended Class.
  • University Degree in Nursing.
  • Two years' experience in a long-term care setting demonstrating strong clinical and assessment skills.
  • Courses specific to gerontology, dementia studies, palliative care/end of life care, and wound care are assets.
  • Working knowledge of Ministry of Health and Long-Term Care regulation standards, Fixing Long-Term Care Act 2021, and relevant legislation.
  • Strong knowledge of computer software: PointClickCare, Excel, Microsoft Word.

Skills and Abilities:

  • Proven time management and organizational skills.

  • Effective oral and written communication skills.
  • Excellent interpersonal skills.
  • Ability to work independently as well as part of a team.
  • Ability to facilitate and adapt to changes in the long-term care environment.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Ability to perform Heimlich Maneuver.
  • Valid Ontario driver's license and access to a reliable vehicle.
  • Computer expertise in corporate standard software (Microsoft Office) and department-specific software to data search and entry and to create and modify word-processed documents.
  • Must be aware of safe work practices as they relate to job responsibilities and work environment, and have the basic understanding of the Occupational Health and Safety Act.
