About the Role:

We are seeking a skilled Registered Practical Nurse (RPN) to join our team as a Community Care Specialist. As an RPN, you will play a vital role in providing high-quality care to patients in their homes, working collaboratively with our team to ensure seamless care transitions.

Key Responsibilities:

  • Conduct scheduled visits in the community to assess patient needs and provide care within your scope of practice.
  • Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure comprehensive care.
  • Provide education and support to patients and families, promoting independence and self-management of care.
  • Collaborate with healthcare professionals to ensure continuity of care and address any concerns or issues that may arise.
  • Maintain accurate and up-to-date patient records, documenting all interactions and care provided.

Requirements:

  • Current registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse in good standing.
  • Valid driver's license and access to a reliable vehicle.
  • Experience in community care or a related field is an asset.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a strong commitment to providing high-quality patient care.
