Job Summary:
We are seeking a highly skilled and compassionate Social Worker I to join our team at Providence Health Care. As a key member of our interdisciplinary team, you will provide therapeutic clinical counselling services to patients/clients, conduct comprehensive social work psychosocial assessments, and develop treatment plans to support their well-being.
Key Responsibilities:- Provide therapeutic clinical counselling services to patients/clients, including exploring their present situation, identifying goals, problems, personal concerns, and treatment objectives.
- Conduct comprehensive social work psychosocial assessments using standardized tools and develop care plans in collaboration with clients/patients.
- Assess patient/client needs and available services, and develop treatment options with patient/client input.
- Facilitate patient/clients' utilization of community services and rehabilitative programs, including researching available services, liaising with service and program providers, and providing advice and consultation to colleagues.
- Maintain patient/client records through electronic methods, documenting assessments, charting information, preparing progress notes and reports in accordance with established standards, policies, and procedures.
- Master's Degree in Social Work from an accredited university.
- Two (2) years' recent, related experience working with diverse marginalized populations in the areas of HIV/AIDS, substance use/addictions, and/or mental health.
- Current full registration with the British Columbia College of Social Workers.
