We are seeking a highly skilled Speech-Language Pathologist to join our team at Home and Community Care Support Services. As a Speech-Language Pathologist, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.
Key Responsibilities- Assess and determine patient care needs and eligibility for speech therapy services.
- Provide access and referrals to community services as needed.
- Engage with patients, caregivers, and other healthcare practitioners to ensure comprehensive care.
- Develop and implement individualized treatment plans to address patient needs.
- Monitor patient progress and adjust treatment plans as necessary.
- Collaborate with multidisciplinary teams to ensure seamless care coordination.
- Maintain accurate and up-to-date patient records.
- Participate in professional development activities to stay current with best practices.
- Master's degree in Speech Therapy and current certification with the College of Audiologists and Speech-Language Pathologists of Ontario.
- Related professional experience, typically acquired over a period of two years in a SLP role.
- Experience in a similar community-based setting is an asset.
- Valid driver's license and access to a vehicle.
- Fluency in English and French is an asset.
- Vulnerable Sector Check completed within the last six months.
