We are seeking a skilled Licensed Practical Nurse to join our hemodialysis team at Fraser Health. As a key member of our interdisciplinary team, you will provide high-quality patient care and work collaboratively with our healthcare professionals to ensure the best possible outcomes for our patients.
Key Responsibilities- Assess, plan, implement, and evaluate patient care during the pre-dialysis, intra-dialysis, and post-dialysis periods.
- Establish and maintain a therapeutic relationship with patients and their families through effective communication and interpersonal skills.
- Contribute to the development and ongoing changes to individualized care plans, including attending multidisciplinary care planning meetings and family conferences.
- Advocate for patients' interests and needs, and escalate care as required for changes in patient condition.
- Perform nursing duties related to hemodialysis in accordance with LPN competencies and Fraser Health standards, policies, and procedures.
- Grade 12, plus graduation from an approved program of Practical Nursing, and successful completion of post-basic education through an in-house program offered by a health authority affiliated with BC Renal.
- Current full practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) and current certificates in CPR Level C or equivalent and First Aid.
