Your Responsibilities:

  • Integrate Indigenous wellness perspectives into patient care plans, advocating for holistic well-being.
  • Coordinate comprehensive care for patients, connecting them with relevant community services and support networks.
  • Champion the needs of patients and families, ensuring their voices are heard and their rights are respected throughout their healthcare journey.
  • Cultivate strong relationships with community agencies and healthcare providers to facilitate seamless transitions and collaborative care.

Your Qualifications:

  • Bachelor's degree in Social Sciences or a related field.
  • Registration with a relevant professional regulatory body.
  • Demonstrated experience working effectively with Indigenous populations.
  • In-depth understanding of psychosocial needs within mental health care, particularly for youth.
  • Familiarity with Indigenous traditional health practices and perspectives.
