Job Summary

The Patient Care Coordinator Mental Health position is a critical role within the Fraser Health Authority, responsible for providing clinical supervision, guidance, and support to nursing staff and other clinical team members. This position requires a strong understanding of mental health principles and practices, as well as excellent leadership and communication skills.

Key Responsibilities
  1. Apply established criteria and prioritize admissions/intake, leading and facilitating actions related to daily patient flow, plans for patient discharge, and appropriate patient placement/referral.
  2. Supervise nursing and nursing support staff, coordinate the work of the clinical team, and monitor the quality of care and safety of patients.
  3. Support quality patient care and excellence in practice by promoting interprofessional collaboration, team building, and staff education.
  4. Initiate/lead/support plan of care conferences and rounds, disseminate clinical information, and provide input into the development of learning objectives.
  5. Participate in quality improvement and risk management activities, and assist the Manager/Director with various tasks and responsibilities.
Requirements
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years of recent related clinical experience in the designated clinical area, or an equivalent combination of education, training, and experience.
  • Clinical proficiency/expertise in the designated clinical area, knowledge of community resources and health-related support services, and ability to supervise and provide leadership and work direction.
