Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Durham Community Health Centre. As a key member of our interdisciplinary team, you will provide comprehensive, client-centered primary health care to community members within high-priority populations.

Key Responsibilities:
  • Provide primary care assessments, screenings, and referrals to community programs
  • Activate clinical pathways and connect clients with community-based and specialty services
  • Work collaboratively with the team to provide clinical services to equity-deserving members of our community
  • Advocate and lead initiatives to champion and support goals that align with Durham CHC's Strategy, Vision, Mission, and Values
Requirements:
  • Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, Primary care
  • Advanced preparation at the Masters level
  • Member of the Nurse Practitioners Association of Ontario (NPAO) or Registered Nurses Association of Ontario (RNAO) is an asset
  • Minimum of three years' experience working as a primary care NP with clients having complex needs
  • Experience working with priority populations, such as Newcomers and ACB communities, is considered an asset
