Nurse Practitioner

Seeking a Nurse Practitioner to provide comprehensive primary healthcare to clients in high-priority neighborhoods through a mobile community-based model.
Working within an integrated team, the successful candidate will provide client-centered care that emphasizes accessibility, health promotion, and continuity of care.
The Nurse Practitioner will perform clinical assessments, therapeutic interventions, and referrals to community programs, while activating clinical pathways and connecting clients with specialty services as needed.
The role requires a strong understanding of the biopsychosocial needs of clients, including those with multiple comorbid health conditions, mental health diagnoses, and substance use/abuse.
The ideal candidate will have a Master's degree in Nursing, a current and valid Nurse Practitioner license, and a minimum of three years' experience working as a primary care Nurse Practitioner.
Key responsibilities include:
- Performing clinical assessments and therapeutic interventions within a community mobile outreach model.
- Participating in client care through a community mobile clinic and performing clinical assessments combined with sound clinical judgement/diagnostic and treatment skills.
- Completing timely and accurate medical records of client encounters, documentation, and third-party correspondence using the Electronic Medical Record (EMR) system.
- Contributing to the development, implementation, and evaluation of medical directives, policies, and procedures, and best-practice guidelines.
Requirements:
- A current and valid Nurse Practitioner license with the College of Nurses of Ontario (CNO).
- Completion of a Master's degree in Nursing.
- Current and valid CPR certification.
- Minimum of three years' experience working as a primary care Nurse Practitioner with adult, children, and youth with complex needs experience.
- Experience working in a community health setting and with individuals who face barriers accessing traditional healthcare services is an asset.
- Flexibility to work in various locations as needed.
- Valid Ontario Driver's license and access to a reliable vehicle.
- Demonstrated ability to communicate with clients in a non-judgmental manner and effectively understand client needs.
