The Foot Care Practitioner will travel between designated homes, delivering essential foot care services to residents and clients. This role is vital in ensuring the health and well-being of those in your care.
Key Responsibilities:
- Provide nursing care in accordance with established standards, physician directives, and organizational policies. Monitor and report on residents' conditions, initiating appropriate health interventions as necessary.
- Conduct assessments of residents and contribute to individualized care plans. Track health changes and determine the need for medical referrals.
- Perform foot care procedures, ensuring adherence to recognized training and competency standards. Document client conditions and any changes accurately and professionally.
Qualifications:
- Advanced and Diabetic Foot Care Certification is mandatory.
- Must be registered or eligible for registration with the Registered Practical Nurses Association of Ontario.
- Possession of a valid 'G' class driver's license and reliable transportation is required.
- Willingness to travel between various work locations.
- Comprehensive understanding of nursing practices and health care protocols, including Infection Control Practices.
- Strong interpersonal skills and a commitment to working with elderly populations.
- Excellent communication abilities and the capacity to work both independently and as part of a team are essential.
