Job Summary

We are seeking a skilled Nurse Practitioner to join our team at North Island Medical Clinic. As a key member of our collaborative team, you will provide wrap-around care to individuals who are marginalized and vulnerable, focusing on low-barrier access to primary care for those struggling with mental health, substance use issues, homelessness, and racism.

Key Responsibilities
  • Provide comprehensive primary care services to patients, including physical and mental health assessments, diagnosis, and treatment.
  • Work collaboratively with a team of healthcare professionals, including Family Physicians, Social Workers, Registered Nurses, and a Clinical Pharmacist, to ensure culturally safe and trauma-informed care.
  • Develop and implement care plans that address the unique needs of patients, including those with complex health issues and social determinants of health.
  • Participate in quality improvement initiatives to reduce barriers to care and improve patient outcomes.
Requirements

To be successful in this role, you will require:

  • Current registration as a Nurse Practitioner with the College of Registered Nurses of British Columbia.
  • Experience working with marginalized and vulnerable populations, including those with mental health and substance use issues.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
