In this role, you will serve as a Community-Based Practical Nurse (RPN), conducting scheduled visits to provide care for patients in diverse environments, with the assistance of Registered Nurses for more intricate cases. You will engage with patients across all age groups, addressing varying levels of care requirements, and play a crucial role in ensuring their comfort at home for as long as feasible. This position is ideal for those seeking autonomy, independence, and a balanced work-life schedule. Key Responsibilities:

  • Conduct scheduled community visits to address patient needs within your professional scope
  • Provide ethical and safe nursing care
  • Utilize appropriate assessment tools to evaluate patients' individual needs
  • Implement and continuously monitor care plans, making adjustments as necessary
  • Engage patients through a collaborative approach, fostering rapport and trust
  • Maintain and update professional patient records related to encounters and care plans

Qualifications:

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver's license and access to a vehicle is essential
  • Experience in community care is advantageous
  • Reliability and adaptability
  • Willingness to travel to patient residences within a designated area
  • Experience working with vulnerable populations
  • A commitment to making a positive impact in the community
  • Availability for on-call coverage, including after-hours, to ensure patient safety and care needs are met
