We are seeking a skilled Registered Practical Nurse to join our team in providing high-quality community care to patients in their homes. As a member of our team, you will have the opportunity to work independently and collaboratively with other healthcare professionals to deliver comprehensive care to patients of all ages and backgrounds.
Key Responsibilities:
- Participate in scheduled visits in the community to assess patient needs and provide care within your scope of practice.
- Provide safe and ethical nursing care, using appropriate assessment tools and techniques to assess patients' individual needs.
- Develop and implement care plans, and continuously monitor and adjust as required.
- Engage patients using a collaborative approach, building rapport and trust to promote positive outcomes.
- Maintain and update professional patient records related to patient encounters and plans.
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability and flexibility.
- Willingness to travel to and from patient residences within a designated area.
- Experience working with vulnerable populations.
- Passion for making a difference in your community.
