This role involves collaborating closely with the Substance Use Connections Team, leveraging your expertise in substance use to deliver essential support to clients.

Key Responsibilities:
  • Perform continuous evaluations of clients' conditions and requirements
  • Deliver extensive bio/psycho/social services to facilitate clients' recovery and functionality
  • Provide case management, counseling, and direct services, including access to vital treatment resources
  • Coordinate access to specialized services and ensure seamless transitions between care levels
  • Build effective relationships with various service providers to optimize treatment resources
  • Document assessments, treatment plans, and progress notes in accordance with professional standards
  • Undertake other related duties as necessary
Scheduling Details:

Shifts are scheduled from 08:30 to 16:30 hours. This position operates on a casual basis, offering flexibility without guaranteed hours.
