Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Fraser Health. As a Social Worker, you will play a vital role in delivering comprehensive social work services to clients and their families in our residential care settings.

Key Responsibilities
  • Conduct thorough psycho-social assessments of clients and their families to identify their emotional, social, and physical needs.
  • Provide clinical counseling services to clients and their families to address issues related to chronic illness, disability, relocation, bereavement, and coping strategies.
  • Collaborate with interdisciplinary teams to develop and implement client care plans that address their unique needs and goals.
  • Facilitate interdisciplinary team conferences and meetings to ensure seamless communication and coordination of care.
  • Provide education and consultation to clients, families, and healthcare professionals on issues related to high-risk factors, adaptation to illness and disability, loss, grief, and bereavement, transition stress, protection of vulnerable adults, and critical incident stress management.
  • Maintain ongoing knowledge of community services available and provide referrals to clients and their families as needed.
  • Contribute to research activities and participate in program planning and development to advance social work practice.
  • Maintain accurate and up-to-date records and reports, including charting on medical charts and workload statistics.
Qualifications
  • Bachelor's Degree in Social Work from an accredited School of Social Work.
  • One year's recent, related experience or an equivalent combination of education, training, and experience.
  • Current full registration with the BC College of Social Workers.
Skills and Abilities
  • Ability to relate to seniors and understand their unique needs and challenges.
  • Knowledge of family systems and dynamics, residential care models, dementia, and emotional and mental health issues related to chronic illness and disability.
  • Knowledge of relevant legislation, including Adult Guardianship Legislation.
  • Counselling and group facilitation skills.
  • Ability to work collaboratively with others, work in a multicultural environment, and understand the frameworks of other disciplines.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to organize and set workload priorities in a complex environment.
  • Ability to accept responsibility, exercise initiative, and work independently.
