About the Role:
The Social Worker II (SW II) Navigation Program is a short-term initiative designed to facilitate continuity of care and connect clients with relevant community services to address their needs. This program operates on a patient-centered, goal-oriented model, focusing on skill-building to promote ongoing independence, reduce distress, and improve client outcomes.
Key Responsibilities:
- Develop and maintain knowledge of community agencies and resources to ensure clients are navigated to appropriate services.
- Organize and sustain a database of resources to support client needs.
- Maintain strong working relationships with key partners and agency representatives to facilitate referrals and support client needs.
Requirements:
- Completion of a bachelor's degree in Social Work.
- Active or eligible registration with the Alberta College of Social Workers (ACSW).
Preferred Qualifications:
- Experience working in mental health and addiction.
