About the Role

We are seeking a highly skilled and compassionate Psychiatric Liaison Nurse to join our team at Fraser Health Authority. As a key member of our interprofessional care team, you will play a vital role in providing person-centered care to patients with psychiatric and concurrent disorders.

Key Responsibilities
  1. Conduct comprehensive psychosocial assessments and develop individualized care plans to address the unique needs of each patient.
  2. Collaborate with emergency department staff, mental health professionals, and community service providers to ensure seamless transitions of care and effective patient flow.
  3. Provide crisis intervention, counseling, and education to patients and their families, promoting empowerment and self-management.
  4. Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
  5. Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practices.
Requirements
  • Graduation from an approved school of nursing or psychiatric nursing.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
  • One year of recent experience in acute or community mental health and substance use settings.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a fast-paced, dynamic environment.
