Job Summary

Fraser Health Authority is seeking a highly skilled and experienced Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for providing clinical supervision, guidance, and support to nursing staff and other clinical team members. Your expertise in patient flow and discharge planning will be instrumental in ensuring the smooth operation of our healthcare services.

Key Responsibilities
  • Supervise and Coordinate Patient Care: Provide clinical supervision and guidance to nursing staff and other clinical team members to ensure high-quality patient care.
  • Manage Patient Flow and Discharge Planning: Develop and implement effective patient flow and discharge planning strategies to optimize patient care and reduce wait times.
  • Lead and Develop Clinical Teams: Lead and develop clinical teams to ensure they have the necessary skills and knowledge to provide high-quality patient care.
  • Collaborate with Interdisciplinary Teams: Collaborate with interdisciplinary teams to ensure seamless communication and coordination of patient care.
  • Monitor and Evaluate Patient Care: Monitor and evaluate patient care to ensure it meets the highest standards of quality and safety.
Requirements
  • Current Practicing Registration: Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Experience and Education: Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training, and experience.
  • Skills and Abilities: Clinical proficiency/expertise in the designated clinical area, ability to supervise and provide leadership and work direction, ability to coordinate patient assignments, and ability to monitor staff workload and patient factors.
