About the Role

We are seeking a highly organized and detail-oriented Care Coordinator to join our team at Qualicare, North York. As a Care Coordinator, you will play a critical role in ensuring the efficient processing of referred clients to our Family Home Care services.

Key Responsibilities
  • Scheduling: Serve as the primary point of contact for inquiries from staff regarding schedules, call-ins, and open shifts, and from clients about all schedule-related matters.
  • Client Services: Respond to client referrals within specific time frames as per our organization's policy, and effectively meet the needs of each client by scheduling them with the caregiving services.
  • Staff Liaison: Manage, verify, and assign schedules daily, and respond to client inquiries and/or problems immediately, as appropriate.
  • HR Support: Assist with recruitment, new staff onboarding, and preparation of the yearly performance evaluation process.
  • Learning and Development: Support staff with online learning and development, and training.
  • Documentation and Communication: Ensure all relevant information is received, and input accurate data into our software package.
  • Quality Management: Respond promptly to incidents, complaints, and compliments, and assess client satisfaction using our established process.
  • Operational Support: Assist with internal organizational projects, software utilization, and office duties as assigned.
  • Community Relations: Promote our services in the community, and attend workshops and seminars to further own knowledge and establish business contacts.
Requirements
  • Minimum of 2-3 years experience with scheduling and/or customer care/client registration.
  • Healthcare experience preferred.
  • Coordinator experience preferred.
  • Proven excellent interpersonal skills and effective oral/written communication skills.
  • Demonstrated excellent organizational, time management, prioritization, problem-solving, and conflict-resolution skills.
  • Ability to work effectively as part of a team, and work with minimal supervision.
Working Conditions
  • 8-hour shift.
  • Day shift.
  • Monday to Friday.
  • Evenings.
  • Weekends.
  • On call.
