Key Responsibilities- Conduct thorough psychosocial assessments to identify patient needs and develop effective care plans.
- Provide individual and group therapy sessions to support patients and their families in coping with illness, injury, or disability.
- Collaborate with healthcare professionals to develop and implement discharge plans, ensuring a smooth transition to community-based care.
- Advocate for patients and their families, addressing social determinants of health and promoting access to community resources.
- Participate in interdisciplinary team meetings to share knowledge and expertise, and contribute to the development of patient-centered care plans.
- Supervise and mentor social work students, promoting their professional growth and development.
- Master's degree in Social Work from a recognized university.
- Registration or eligibility for registration with the Nova Scotia College of Social Workers.
- Two years of clinical experience as a social worker, preferably in a healthcare setting.
- Strong counselling and communication skills, with the ability to work effectively with diverse patient populations.
- Capacity to work autonomously and collaboratively within an interdisciplinary team.
- Knowledge of social determinants of health, and ability to advocate for patients and their families.
This is a long-term assignment, with a full-time position requiring 75 hours of work per bi-weekly period. The successful candidate will be required to work in a dynamic and fast-paced environment, with a focus on providing high-quality patient care.
Compensation and BenefitsThe Nova Scotia Health Authority offers a competitive salary range of $37.31 - $49.05 per hour, depending on experience. Successful candidates may also be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
