Job Summary:

Closing the Gap Healthcare Group is seeking a skilled Speech Language Pathologist to join our team as a Home Care Specialist. As a Speech Language Pathologist with our company, you will work independently with adult clients in their home settings, providing face-to-face therapy services.

Key Responsibilities:

  • Conduct assessments in home settings to identify presenting impairments or diagnoses using standardized assessment tools.
  • Develop and implement care plans using evidence-based information, relevant best practice guidelines, and clinical expertise.
  • Collaborate with clients' family members, caregivers, and other healthcare professionals to ensure comprehensive care.

Requirements:

  • Graduate of a recognized post-secondary program in Speech Language Pathology
  • Current active registration in good standing with the College of Audiologists and Speech-Language Pathologists of Ontario
  • Valid driver's license and reliable vehicle
  • Excellent communication and analytic skills, with strong organizational abilities
