We are seeking a Registered Practical Nurse (RPN) to join our team in a permanent full-time night rotation position. The successful candidate will be responsible for providing high-quality care to our residents, working collaboratively with the interdisciplinary team to ensure seamless care delivery.
Key Responsibilities- Assist the Unit Manager in completing and ensuring resident assessments are accurate and up-to-date
- Plan, coordinate, manage, and evaluate resident care plans in collaboration with the resident, substitute decision maker, physician, nursing team, and contracted service providers
- Process and follow orders from the Medical Director and/or Nurse Practitioner
- Assess and accurately report resident response to care in a timely manner
- Prepare for scheduled care conferences and participate in conferences as applicable
- Respond to resident/family concerns and ensure appropriate action is taken and/or concerns communicated to the Unit Manager and/or Director of Care
- Responsible for the safe and accurate administration of medications and treatments, assessing and documenting response to treatment, and reporting any incidents related to medication administration to the Unit Manager and Medical Director
- Provide education and support to residents and families by maintaining an open, knowledgeable rapport, communicating changes in resident condition as necessary, and adhering to privacy and confidentiality protocols
- Respond to and manage significant changes in resident condition and/or medical emergencies, providing emergency first aid
- Provide supervision and direction to Personal Support Workers (PSWs) while promoting a multidisciplinary team approach to care and service
- Assist in evaluating the performance of PSWs annually and as needed
- Current registration as a Registered Practical Nurse in the province of Ontario
- Experience working in a long-term care environment and with unregulated care providers preferred
- Strong written and oral communication, critical thinking, care coordination, time management, and customer focus skills
- Demonstrated specific knowledge/skill in current geriatric nursing care and standards preferred
- Computer proficiency in PointClickCare and experience with MDS-RAI or other electronic documentation system required
- Wound care experience required
- Competitive hourly rate: $27.12-$28.62
