Job Summary

We are seeking a highly skilled and compassionate Outreach Worker to join our Overdose Outreach Team (OOT) at Fraser Health. As a key member of our interdisciplinary team, you will provide critical support services to individuals and their families affected by substance use issues.

Key Responsibilities
  • Monitor clients' health conditions and environments, documenting changes and reporting significant observations to the Registered Nurse or delegate.
  • Arrange and accompany clients on medical appointments and errands, providing emotional and physical support to promote self-management skills.
  • Create linkages to community resources and programs, assisting clients with accessing benefits and providing advocacy as needed.
  • Administer medication to clients under the direction of the Registered Nurse and provide medication reminders.
  • Identify potential emergency situations and make appropriate interventions, such as performing cardiopulmonary resuscitation or contacting emergency services.
  • Act as a liaison between agencies, updating team staff on available services and liaising with community agencies to provide information.
  • Provide basic health-promotion information and promote life-skills development in clients, encouraging empowerment and participation in healthcare decisions.
  • Perform general clerical duties, including responding to inquiries, maintaining client files, and producing documents.
Qualifications
  • Completion of a Substance Use Certificate and one year's recent experience working with clients with substance use issues, or an equivalent combination of education and experience.
  • Valid BC driver's license and vehicle.
  • Current CPR certificate.
Requirements
  • Demonstrated ability to work with marginalized and culturally diverse populations.
  • Excellent listening and communication skills.
  • Ability to work independently and collaboratively in a team environment.
  • Physical ability to carry out the duties of the position.
