Job Title: RPN - Primary Health Care Specialist

We are seeking a highly skilled Registered Practical Nurse (RPN) to join our team at Guelph CHC. As a Primary Health Care Specialist, you will play a vital role in providing high-quality care to our clients, particularly those from marginalized communities.

Job Summary:

The successful candidate will be responsible for providing nursing assessments, developing care plans, and implementing interventions to promote health and prevent disease. You will work collaboratively with our interdisciplinary team to ensure seamless care delivery and excellent client outcomes.

Key Responsibilities:
  1. Conduct comprehensive nursing assessments of clients receiving clinical care and develop individualized care plans.
  2. Perform phlebotomy and collect other specimens for testing, as well as treat STIs in accordance with medical directives.
  3. Order basic blood, urine, and STI tests, as per medical directives, and respond to client medical needs and requests in the absence of a physician.
  4. Collaborate with other health and social service providers to ensure comprehensive care and support for clients.
  5. Provide education and information to clients to promote health and prevent at-risk behaviors.
  6. Respond to urgent or crisis situations, including emergency codes.
Requirements:
  1. Registered Practical Nurse degree and membership in the College of Nurses.
  2. Three to five years of broad-based practical nursing experience in a combination of hospital and community settings and/or public health.
  3. Knowledge and experience working with clients with HIV/AIDS and a commitment to anti-oppressive practice using a harm reduction approach.
  4. Demonstrated commitment to and knowledge of community-based health care, the social determinants of health, and health equity.
  5. Basic word processing and computer proficiency.
  6. Ability to set goals and get results within a set timeline.
  7. Ability to multitask and work independently.
  8. Effective written and oral communication skills.
  9. Effective time management, decision-making, and organizational skills.
  10. A good team member and supportive of interdisciplinary practice.
  11. High level of confidentiality.
  12. Flexible toward change.
Working Environment:

The successful candidate will work primarily in a well-lit and ventilated clinic with spacious and well-laid-out exam rooms. The role will involve 70% client interaction and 30% desk work, meetings, and frequent changes in position. Some travel by car may be required.
