At Vision Loss Rehabilitation Canada, we are seeking a highly skilled and compassionate Client Care Navigator to join our team. As a key member of our team, you will play a vital role in coordinating and integrating supports and services around individuals living with vision loss.
Key Responsibilities:- Assess the needs of individuals with vision loss and develop personalized care plans to achieve their goals.
- Coordinate and facilitate linkages between clients, healthcare providers, and community services to ensure seamless care.
- Provide education and support to clients and their families to enhance their understanding of vision loss and available resources.
- Collaborate with multidisciplinary teams to ensure comprehensive care and address emerging needs.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
- 3-5 years of experience in client service, preferably in the health and social service sectors.
- Experience working directly with individuals with vision loss and their families.
- Knowledge of available community services and clinical, social, and residential care options.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Proficiency in technology, including Microsoft Office and case management systems.
