Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Fraser Health. As a key member of our Home Health Services team, you will play a vital role in providing high-quality patient care and support in the comfort of clients' homes.

Key Responsibilities
  • Conduct initial client consultations, assessments, and interventions to determine client needs and develop personalized care plans.
  • Screen referrals and confirm client eligibility for services, ensuring timely and efficient access to care.
  • Prepare and maintain accurate client records, providing essential data and statistics for team planning and quality improvement.
  • Collaborate with interdisciplinary teams to coordinate referrals, prioritize services, and ensure seamless care transitions.
  • Provide education and counseling to clients, families, and caregivers, promoting health literacy and self-management skills.
  • Communicate effectively with healthcare professionals, organizations, and community resources to ensure comprehensive care and support.
  • Maintain and complete required documentation, compiling information and data as needed to inform care planning and quality improvement initiatives.
Requirements
  • Graduation from an approved Practical Nursing program, plus one year of recent related acute, residential, and/or community-based experience.
  • Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Certificates in CPR Level C or equivalent and First Aid.
