Job Summary

We're seeking an experienced Occupational Therapist to join our team in the Community Access role. As an Occupational Therapist with Champlain Rehab Solutions, you'll have the opportunity to work with diverse client groups across Southern Ontario and the capital region, providing consultation, assessment, and planned interventions that promote independence and well-being.

Key Responsibilities
  • Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards.
  • Develop and implement client-centered care plans that address the unique needs and goals of each client.
  • Collaborate with multidisciplinary teams to ensure coordinated delivery of care and effective communication.
  • Document all assessment findings, treatment plans, progress, and discharge summaries in accordance with professional standards and organizational requirements.
Requirements
  • Master's degree in Occupational Therapy from a recognized school of Occupational Therapy.
  • Member in good standing with the College of Occupational Therapists of Ontario.
  • Valid Driver's License with access to a vehicle.
  • Compassionate and empathetic approach to client care.
  • Ability to work independently and as part of a team.
  • Strong communication and documentation skills.
