About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our NSWOC team at CarePartners. As a NSWOC Nurse, you will provide expert wound and ostomy care to patients in their homes and communities.

Key Responsibilities
  • Assess and manage wounds and ostomies, providing individualized care plans to promote optimal patient outcomes.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
  • Provide education and support to patients and caregivers on wound and ostomy care, promoting self-management and independence.
  • Stay up-to-date with the latest research and evidence-based practices in wound and ostomy care, applying this knowledge to improve patient outcomes.
Requirements
  • Current College of Nurses of Ontario (CNO) registration in good standing.
  • Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending.
  • Several years of escalating experience managing wounds and ostomies.
  • Strong desire and commitment to making a difference in the lives of our patients.
  • Exceptional interpersonal skills.
  • A valid driver's license and vehicle, as regular travel is required.
  • Excellent English verbal and written communication skills.
  • CPR Certification.
  • Current immunization record, including 2-step TB test.
  • To protect our patients, we require a current vulnerable sector check and a clear background check.
