As a key member of our interdisciplinary team, you will provide primary health care to our patients, working collaboratively with our family physicians to ensure comprehensive and holistic care.

Key Responsibilities:
  • Provide comprehensive health assessments and develop health plans in collaboration with patients and other healthcare professionals.
  • Order and interpret diagnostic tests, prescribe medications, and provide counseling on health promotion and illness prevention strategies.
  • Work collaboratively with our family physicians to ensure coordinated and comprehensive care for our patients.
  • Participate in patient education and health promotion activities, including workshops and community events.
  • Develop and maintain relationships with community partners and other healthcare providers to ensure seamless care for our patients.
Requirements:
  • Current registration as a Registered Nurse in the extended class with the Colleges of Nurses of Ontario.
  • Current CPR and BCLS certification.
  • Knowledge of primary health care and the social determinants of health.
  • Experience in program development, implementation, monitoring, and evaluation is an asset.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
