Key Responsibilities:

  • Oversee and execute best practices within the Resident Assessment Instrument-Minimum Data Set (RAI-MDS) framework.
  • Collaborate with an interdisciplinary team to ensure comprehensive resident care.
  • Deliver training and educational sessions on the RAI MDS processes.
  • Maintain exceptional standards of resident care while adhering to established protocols.
  • Monitor, document, and report vital signs and symptoms accurately.
  • Organize and lead initiatives related to resident care management.
  • Support the supervision and development of nursing staff.

Qualifications:

  • A minimum of 3-5 years of experience in Long-Term Care settings.
  • Current registration with the College of Nurses of Ontario (CNO) in good standing.
  • Proficient knowledge in nursing rehabilitation practices and collaboration with physiotherapy.
  • Strong analytical skills and ability to conduct thorough assessments.
  • Experience in leading training sessions for adult learners.
  • Familiarity with the InterRAI suite of assessment tools and patient classification systems.
  • Excellent communication skills, both verbal and written.
