Registered Practical Nurse (RPN) - RAI Coordination
Are you dedicated to enhancing the quality of care for residents in long-term care settings? We invite you to explore an opportunity with Southbridge Care Homes as a Registered Practical Nurse focused on RAI coordination. Your role will be pivotal in ensuring that our residents receive the highest standard of care.
Key Responsibilities:
- Oversee and execute best practices within the Resident Assessment Instrument-Minimum Data Set (RAI-MDS) framework.
- Collaborate with an interdisciplinary team to ensure comprehensive resident care.
- Deliver training and educational sessions on the RAI MDS processes.
- Maintain exceptional standards of resident care while adhering to established protocols.
- Monitor, document, and report vital signs and symptoms accurately.
- Organize and lead initiatives related to resident care management.
- Support the supervision and development of nursing staff.
Qualifications:
- A minimum of 3-5 years of experience in Long-Term Care settings.
- Current registration with the College of Nurses of Ontario (CNO) in good standing.
- Proficient knowledge in nursing rehabilitation practices and collaboration with physiotherapy.
- Strong analytical skills and ability to conduct thorough assessments.
- Experience in leading training sessions for adult learners.
- Familiarity with the InterRAI suite of assessment tools and patient classification systems.
- Excellent communication skills, both verbal and written.
