Job Title: Case Coordinator

About the Role:

We are seeking a highly organized and detail-oriented Case Coordinator to join our team at pt Health. As a Case Coordinator, you will play a critical role in reviewing referrals, scheduling medical evaluations, and ensuring seamless communication with our assessors and clients.

Key Responsibilities:

  • Review referrals to ensure suitability for medical evaluation
  • Schedule medical evaluations with our roster of assessors
  • Coordinate secondary services, such as transportation and interpretation
  • Produce notifications and directives for assessors
  • Ensure data accuracy and meet demanding timelines

Requirements:

  • Clinical background, preferably as a registered health professional
  • Strong customer service orientation
  • Attention to detail and proficiency in systems
  • Mature work ethic and computer efficiency
  • Competent verbal and written communication skills in English
  • Proven organizational and multi-tasking skills
  • French-English bilingualism is a significant asset
