About Our Opportunity

We are seeking a highly skilled and compassionate Care Coordinator to join our team at The Government of Nova Scotia. As a key member of our Disability Support Program, you will play a vital role in determining eligibility for funding, conducting comprehensive needs assessments, and connecting individuals with the resources they need to live fulfilling lives.

Primary Accountabilities
  • Determine Eligibility: Review and analyze various sources of information to determine eligibility for Disability Support Program funding, applying our eligibility policy and leveraging supportive decision-making practices to empower applicants.
  • Conduct Needs Assessments: Collaborate with healthcare professionals to conduct comprehensive, multidimensional needs assessments for individuals and families who meet DSP funding criteria.
  • Allocate Funding: Utilize DSP frameworks such as the Service Group Framework to allocate personalized funding, ensuring clear and empathetic communication with DSP participants.
  • Support Ongoing Funding Requests: Conduct reviews as necessary, ensuring alignment with DSP policies and thresholds.
  • Cultivate Positive Relationships: Foster positive relationships with colleagues, DSP participants, service providers, and the broader community.
Qualifications and Experience

We are seeking a candidate with a strong background in a related field, such as nursing, social work, occupational therapy, physiotherapy, or nutritional studies. A minimum of five years of related experience is required, with a valid driver's license and access to reliable transportation. You must be currently registered, or eligible for registration, with your applicable professional association.

  • Experience in Eligibility Determination: Proven experience in eligibility determination and assessment processes within social services or disability support programs.
  • Needs Assessment and Interpretation: Experience conducting multidimensional needs assessments and interpreting assessment results to inform funding decisions.
  • Strong Analytical and Critical Thinking Skills: Ability to review and evaluate complex information, with strong analytical and critical thinking skills.
  • Excellent Interpersonal and Communication Skills: Ability to communicate effectively and empathetically with diverse stakeholders, with strong interpersonal and communication skills.
