Job Summary

The Nutrition Care Specialist at Prairie Mountain Health is responsible for providing high-quality nutritional care and counseling to patients, residents, and clients. This role is a key member of the healthcare team, working collaboratively with patients, residents, clients, and next of kin to develop and implement effective nutrition care plans.

Key Responsibilities:

  • Conduct comprehensive nutrition assessments to identify individual needs and develop personalized care plans.
  • Collaborate with healthcare teams to provide nutrition education and counseling to patients, residents, and clients.
  • Develop and implement nutrition care plans that align with current best practice evidence and standards of practice.
  • Maintain accurate and up-to-date records of patient care, as per established standards.
  • Provide nutrition expertise to support program-specific procedures and initiatives.
  • Participate in multidisciplinary team meetings, including care conferences, clinical rounds, and discharge planning.

Team Collaboration:

  • Participate in team meetings to develop program action plans, nutrition care-related policies, and procedures.
  • Contribute to sub-committees as required, sharing expertise and knowledge to support team goals.
  • Report on relevant issues and concerns from clinical areas, providing valuable insights to inform decision-making.
  • Share knowledge and expertise with team members to enhance overall performance and quality of care.
  • Prepare and submit statistical and status reports to the Area Manager, providing valuable insights into program performance.
