Job Summary

Key Responsibilities
  • Provide holistic patient care based on advanced nursing and basic medical management knowledge and skills for a selected patient population.
  • Assess patients' needs and develop individualized care plans to promote optimal health outcomes.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination.
  • Manage complex chronic diseases, including pain and symptom management, and prevent unnecessary hospitalizations.
  • Participate in quality improvement initiatives and contribute to the development of standards for case management.
Requirements
  • Membership in good standing with the College of Nurses of Ontario.
  • Registered Nurse in the Extended Class (RN EC designation).
  • Working knowledge of community resources and roles of healthcare professionals.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
Preferred Qualifications
  • Specialized education in palliative pain and symptom management.
  • Palliative/end-of-life care experience.
  • Awareness of Medical Assistance in Dying Legislation.
  • Complex chronic disease management and psychogeriatric care experience.
  • Advanced assessment skills and sound knowledge of clinical therapeutics.
