Position Overview

Join Vancouver Coastal Health (VCH) as a Counsellor specializing in Concurrent Disorders within our Integrated Care Team.

VCH is seeking a dedicated Counsellor to contribute to our multidisciplinary STEPS Strathcona Team Rehabilitation Program. This role is vital in supporting individuals and families facing various health challenges.

Key Responsibilities:

  • Address the care needs of clients and their families through a collaborative, integrated health service approach.
  • Adhere to VCH policies, guidelines, and clinical practice standards.
  • Coordinate care plans across multiple healthcare settings, including primary care, mental health, and community services.
  • Support clients dealing with complex health issues such as chronic diseases, mental health concerns, and social determinants impacting their well-being.
  • Utilize health management principles to identify and resolve issues in collaboration with the interdisciplinary team.
  • Serve as a resource for the Integrated Care Team, providing insights based on theoretical and practical knowledge of complex health needs.
  • Engage in direct care activities, including assessment, planning, implementation, evaluation, and documentation of client care.
  • Build and maintain therapeutic relationships grounded in respect and understanding.

Qualifications:

Education & Experience:

  • Master's Degree in a behavioral health discipline (e.g., counselling, psychology, social work) from an accredited institution, including a clinical practicum, along with two years of relevant experience.
  • Possession of a valid BC Driver's License is preferred.

Knowledge & Skills:

  • Thorough understanding of various healthcare disciplines and their roles in client care.
  • Proven ability to navigate diverse situations requiring initiative and professional judgment.
  • Familiarity with client and family-centered recovery models in mental health.
  • Competence in care coordination for a designated caseload.
  • Ability to assess risk factors and knowledge of relevant legislation affecting client rights.
  • Experience in providing culturally safe care, particularly for Aboriginal populations.
  • Knowledge of Trauma-Informed Practice, Harm Reduction, and Recovery-Oriented Care.
  • Extensive knowledge of crisis intervention and supportive counseling techniques.
  • Strong communication skills, both verbal and written, to effectively engage with clients, families, and healthcare professionals.
  • Demonstrated ability to prioritize workloads and adapt to changing circumstances.
  • Capacity to work independently and collaboratively within a multidisciplinary team.
  • Ability to provide leadership and consultation as needed.
  • Proficient in utilizing technology for client care documentation and communication.
