Qualifications
Education & Experience
- Active registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Minimum of two years' recent experience in case management or community nursing
- Possession of a valid BC Driver's License is mandatory
Knowledge, Skills & Abilities
- Proven ability in conducting comprehensive, evidence-based health evaluations
- Capacity to involve clients and their support network in the decision-making process
- Understanding of both acute and chronic illness management
