Job Title: Registered Nurse - Chronic Disease Education

Job Summary:

The Registered Nurse - Chronic Disease Education is an integral member of a collaborative interdisciplinary primary health care team. The successful candidate will be responsible for providing prevention, education, care, and support services for clients of the Diabetes and Heart Health Program.

Key Responsibilities:
  • Support clients in self-management of chronic disease.
  • Apply the nursing process by assessing client needs, developing and implementing a plan of care, and evaluating outcomes.
  • Employ health promotion, health education, motivational interviewing, and harm reduction strategies to support behavioral changes conducive to health.
  • Provide group education presentations and facilitation activities.
  • Assess for risk factors for developing chronic diseases and facilitate referrals as appropriate.
  • Provide care that is culturally competent and respectful of diversity.
  • Practice as a member of a collaborative interdisciplinary team, including liaison, consultation, routine and priority reporting, and referral as required.
  • Collaborate with partners to promote and support the adoption of clinical practice guidelines for the management of chronic disease management.
  • Review client medications in accordance with regional policy.
  • Identify and address client learning needs and readiness.
  • Practice is consistent with professional standards of practice, codes of ethics, and current knowledge.
  • Complete documentation according to nursing standards and PMH regional and program policies and guidelines.
  • Encourage maximum independence and accountability for self-care according to the client's capacity.
  • Acquire and maintain a comprehensive understanding of health/social services and referral processes.
  • Respond appropriately in crisis situations.
  • Prioritize workload effectively.
  • Complete administrative requirements as required.
  • Protect the confidentiality of information in accordance with the Personal Health Information Act.
  • Monitor and order necessary supplies and medications.
  • Ensure clinical equipment is in working order and address any malfunctions or deficits.
  • Participate in the orientation and training of new staff.
  • Provide supervision, mentorship, orientation, and teaching opportunities to students.
  • May be asked to assist other programs in accordance with provincial protocols and regional policies and procedures.
  • Participate in research activities and utilize results in evidence-informed practice.
  • Inform manager of any unsafe acts, working conditions, incidents, near misses, injuries, or illnesses, workload issues, issues of a political nature, or media interest.

Requirements:

The successful candidate will possess a Bachelor of Science in Nursing degree and be registered with the College of Registered Nurses of Manitoba. The candidate will also have experience working in a primary health care setting and knowledge of chronic disease management.
