We are seeking a highly skilled and compassionate Social Worker I to join our Acute Care team at Fraser Health. As a key member of our interdisciplinary team, you will provide comprehensive therapeutic clinical counselling services to patients and families in our Acute Care setting.
Key Responsibilities- Conduct comprehensive social work and psycho-social assessments of patients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices.
- Provide leadership to the interdisciplinary team in care planning, including discharge planning and hospital stay, by interviewing patients and families to assess and determine care plans and types of services required.
- Collaborate with hospital and community services to determine alternate levels of care and the effective utilization of patient resources.
- Communicate with community health and social agencies, including government ministries, continuing care personnel, and family physicians, to assess and manage individuals at risk and make referrals to appropriate resources.
- Act as a resource to patients and their families by providing information on community resources and access to these resources, suggesting contacts to facilitate optimal planning, arranging referrals, and facilitating the development of community resources to support the health of families.
- Masters Degree of Social Work from an accredited school of Social Work.
- Current full registration with the BC College of Social Workers.
- Ability to initiate, develop, and carry out research activities.
- Demonstrated ability to communicate effectively both verbally and in writing with patients, families, staff, and community agency personnel.
- Demonstrated ability to organize and prioritize workload in a complex environment.
