The primary role of the Clinical Program Coordinator is to support patients, caregivers, and the community in managing health conditions and transitions, while enhancing the patient's self-care capabilities. This position fosters a culture of caregiver involvement in the patient's care plan.
The Clinical Program Coordinator handles incoming and outgoing calls, communicates messages, and guides patients' families and visitors. Responsibilities also include completing and processing diagnostic requisitions, initiating transfer documentation, and managing physician orders. The coordinator is responsible for accurately filling out, sending, receiving, delivering, and filing requisitions on patient records, ensuring that all charts are comprehensive. Additionally, the role involves ordering necessary supplies for the unit.
Other tasks may be assigned as needed.Job Requirements
- Completion of Ontario Secondary School Diploma
- Proficiency in Medical Terminology (with a minimum passing score of 75%)
- Medical Office Assistant Course or equivalent experience
- Familiarity with Word, StaffRight, Excel, Anzer
- Solid understanding of Emergency Response Codes
- Capability to multitask and perform under pressure
- Strong interpersonal skills to interact effectively with patients and their families
- Proven ability to process physician orders accurately
- Competence in reading and comprehending instructions, memos, etc.
- Ability to perform basic arithmetic for report completion
- Capacity to meet the physical demands of the role
- Typing speed of 45 words per minute
- Excellent organizational skills and a strong team-oriented mindset
- Demonstrated regular and punctual attendance
- Commitment to ensuring patient safety
- Functional proficiency in both official languages (oral and written in English; oral in French – testing completed)
