Key Responsibilities:
  • Assess patient care needs and develop individual care plans
  • Link patients with service providers and coordinate care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources
Requirements:
  • Membership in good standing with the applicable regulatory body
  • Three to five years of recent experience in children's health with a focus on children and youth with medical complexity
  • Understanding of Ontario's provincial supports and strategies available to children with medical complexity
  • Experienced and knowledgeable regarding community services and resources for children and youth within Mississauga Halton Geography
  • Clinical expertise providing patient and family-centred care
  • Leadership abilities to support inter-professional teams
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
