We are seeking a highly skilled Registered Nurse to join our team at Fraser Health. As a key member of our inter-professional care team, you will provide direct patient care, assess patient needs, and develop and implement individualized care plans.
Key Responsibilities- Conduct comprehensive nursing assessments of patients and unborn babies in outpatient clinics and home environments.
- Collect and analyze data from various sources to inform nursing diagnoses and develop effective care plans.
- Collaborate with physicians and other healthcare professionals to identify and resolve patient care issues.
- Develop and implement individualized care plans, including education and support for patients and families.
- Provide individualized counseling, group facilitation, and lectures to patients and families.
- Assess learning needs of patients and families, and provide education and support to promote health and well-being.
- Document patient assessments, nursing diagnoses, interventions, and outcomes in accordance with practice standards and Fraser Health procedures.
- Participate in quality improvement and risk management activities, and identify opportunities for process improvement.
- Act as a preceptor or mentor to new nursing staff and students, promoting the philosophy of family-centered care and best practice principles.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.
- Two years of recent related clinical experience in the designated clinical area, and completion of post-graduate coursework and/or certification applicable to the designated clinical area.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes as required.
- Clinical proficiency in the designated clinical area.
- Ability to complete initial and ongoing patient assessments, and apply nursing theory and expertise to provide nursing care through appropriate and prescribed technical, therapeutic, and safety interventions.
- Ability to recognize acuity changes and use factual information, prior learning, and basic principles and procedures to support nursing decisions and actions.
- Ability to communicate effectively with patients, families, caregivers, and members of the inter-professional care team.
- Ability to promote person-centered care, demonstrate empathy for patients and families, and advocate for patient rights.
- Ability to integrate and evaluate data from multiple sources to problem-solve effectively, and plan, organize, and prioritize work in a continually changing environment.
