This is a permanent position working 75 hours bi-weekly, establishing a 12-month eligibility list that may be used to fill various positions.
The successful candidate will be based in Whitehorse, Yukon, and will be part of the Yukon Home Care Program, providing client-focused health services in the home.
Key Responsibilities- Provide professional care for residents/clients by promoting their physical, emotional, and spiritual well-being.
- Lead, coach, and train team members to ensure high-quality care.
- Develop and implement care plans, assessing, documenting, and evaluating interventions and supports provided.
- Collaborate with clients, families, and healthcare teams to ensure comprehensive care.
- Eligible for registration as a Licensed Practical Nurse.
- Recent knowledge of nursing theory, practices, and techniques.
- Experience in long-term care and/or community care environments is an asset.
- Excellent communication, organizational, and critical thinking skills.
- Ability to work collaboratively within a transdisciplinary environment.
- Leadership and coaching skills to support team members.
This role requires shift work, exposure to plants and/or flowers, and heavy lifting (50lbs and over).
