Key Responsibilities:
- Deliver community-based Occupational Therapy services in clients' homes.
- Conduct thorough assessments using various techniques to evaluate home safety, mobility, cognition, and more.
- Collaborate with clients and caregivers to develop and implement evidence-based intervention plans.
- Provide education and resources to clients and their families, linking them to community services as needed.
- Maintain accurate documentation of assessments, care plans, and progress in accordance with professional standards.
Qualifications:
- Registered Occupational Therapist with the College of Occupational Therapists of Ontario.
- Experience in community settings is preferred, though new graduates are encouraged to apply.
- Strong ability to work independently and collaboratively within an inter-professional team.
- Excellent organizational, communication, and report-writing skills.
- Proficient in using technology and software relevant to the role.
- Reliable transportation is essential for this position.
