Job Title: Occupational Therapist

Community Advantage Rehabilitation (CAR), a division of Lifemark, is seeking a qualified Registered Occupational Therapist to provide services to our Adult clients within the community.

Job Summary:

We are looking for a skilled Occupational Therapist to join our team and provide community-based rehabilitation services to adults. The ideal candidate will have experience working in a community setting and be able to work independently and as part of an inter-professional team.

Key Responsibilities:
  • Provide ministry-funded, community-based Occupational Therapy services within the client's place of residence
  • Conduct occupational therapy assessments and develop and implement best practice and evidence-based occupational therapy interventions
  • Provide education to clients and caregivers and link to community agencies and resources when appropriate
  • Complete funding applications and maintain documentation related to assessment findings, care plans, progress, and discharge summaries
Requirements:
  • Registered Occupational Therapist with the College of Occupational Therapists of Ontario
  • Experience working in a community setting is preferred but will consider new graduates
  • Ability to work independently and as part of an inter-professional team
  • Excellent oral, written, and report writing skills
  • Proficiency with computers and strong interpersonal and organizational skills
