Registered Nurse/Registered Psychiatric Nurse, Hospice Residence

Fraser Health is seeking a skilled Registered Nurse/Registered Psychiatric Nurse to join our team at Melville Hospice (Peace Arch Hospital) in White Rock, BC. As a member of our multidisciplinary team, you will provide direct nursing care to patients in a hospice residence setting.
Responsibilities- Assess, plan, implement, and evaluate nursing care for assigned patients, ensuring their physical, emotional, spiritual, and psychosocial needs are met.
- Collect and analyze patient data to inform care decisions, collaborating with the healthcare team to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and patient-centered care.
- Advocate for patients' rights, dignity, and autonomy, promoting a safe and respectful environment.
- Document patient information accurately and timely, using electronic health records to track progress and outcomes.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.
- Maintain a safe and healthy work environment, adhering to established safety protocols and procedures.
Education and Experience:
Graduation from an approved School of Nursing and/or Psychiatric Nursing, with a minimum of two years' recent experience in acute care, including one year in a hospice/palliative care environment.
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills and Abilities:
- Knowledge of nursing practice in hospice palliative care
- Ability to work independently and collaboratively as part of a multidisciplinary team
- Effective communication and interpersonal skills
- Organizational and time management skills
- Ability to prioritize tasks and manage multiple responsibilities
